We are seeking a Nurse Practitioner to serve as an inpatient Palliative Care consultant within local area hospitals to provide excellent patient care through optimized symptom management and complex discussions surrounding goals of care or advance care planning. Hospice or palliative care experience is strongly suggested, as well as a desire to independently build a strong referral base within the assigned facility. JOB SUMMARY The Nurse Practitioner works independently, and in collaboration with, his/her Alive supervising physician to deliver excellent palliative care alongside the care of the hospital-based medical team. The NP provides palliative medical services by managing the symptoms related to acute and chronic conditions, assisting with communication and coordination of care, as well as developing treatment plans. This includes collaboration with the medical team, social workers, case managers, chaplains, etc to optimize holistic care of the patient. The NP would provide recommendations for and/or orders including pharmaceutical interventions/medications, laboratory and diagnostic services as needed, and will work closely with the patient and family to provide disease specific education and to establish goals of care. Due to a referral-based census, the NP will exhibit excellent customer service and professionalism to all patients and referral sources, as well as ambition to build and grow referral source relationships. The NP must have a positive attitude, display comfort in autonomy, demonstrate initiative and flexibility, and be team-oriented. Required Licensed Advanced Practice Provider in the State of Tennessee Certified as an Advanced Practice Provider by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC). Medicare billing privileges or eligibility for privileges Have insurance required by state law to practice and prescribe in the state where services are provided, or be eligible for such insurance requirements to be satisfied by group coverage with company Current driver's license and automobile insurance as required by Tennessee State Law One (1+) year experience as a Nurse Practitioner. Three (3) to five (5) years total nursing experience. Preferred Hospice, palliative, and/or geriatric practice experience Board Certification as an Advanced Certified Hospice and Palliative Nurse (ACHPN) Inpatient/Hospital-based experience as an NP or RN ICU experience Experience in developing a referral-based census Knowledge of disease-specific criteria for hospice eligibility PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
