Education and Training: Graduate from an approved school of nursing Licensed to practice in this state. Participate in continuing education so that she has lmowledge of current trends and developments in nursing and maintains license. Job Knowledge and Capabilities: Ability to learn, absorb, and apply professional training; follow doctor’s orders; and keep records in prescribed manner. Ability to perceive changes in patient behavior and recognize symptoms and the ability to communicate effectively in writing, by telephone, and in personal contact. Ability to organize work procedures, assume responsibility,and tactfully supervise others. * Special interest in geriatric nursing. Specific Duties and Responsibilities Required: Assure that all staff have been assigned duties. Make rounds on each wing, with specific, attention to high acuity patients. Monitor each wing to ensure that appropriate nursing care (according to established policies and procedures) is being provided and that doctors and families are being notified of changes in patients’ condition. * Assume responsibility for assisting with meds and treatments as needed ( including IV fluids and IV meds). Assist with admitting patients as required. Assist in recruiting, hiring, and orientation of new staff. Maintain the self-respect, personal dignity, and physical safety of each patient. Assist patients to attain highest level of self-care possible. Assist physician during rounds. Supervise LPNs. Do performance evaluations as required (two times per year). Make changes in staff schedule as indicated by absence and illness and contact additional help to obtain their services where needed. Carry out personnel disciplinary actions(s): written verbal warnings, suspension, and termination) promptly and effectively as stated in personnel policies and amendments. Maintain the s??curity of the premises and direct unauthorized persons coming on the premises to leave. * Perform other duties as assigned. Relieve other shifts if needed. Willing to perform duties as a CNA when needed. Purpose: The LPN is considered to be a supervisor and is required to exercise supervisory authority over all non­licensed nurses. The exercise of supervisory authority by LPNs is required even at times that RNs are on duty. While the LPN may seek guidance from the RN in a particular situation, he or she is expected to independently issue discipline, evaluate employees and perform other supervisory responsibilities. Specific Patient Care Duties and Responsibilities: Assume responsibility for safe administration of medications and treatments. Coordinate nursing activities with the activities of other departments. Admit new patients. Make frequent rounds to verify that appropriate nursing care (according to established policies and procedures is provided; note any changes in a patient’s condition; intervene to correct problems observed. Assist physician during rounds. Maintain the self-respect, personal dignity, and physical safety of each patient. Assist patients to attain highest level of self-care possible. Relieve other shifts if needed. Perform other duties as assigned. Specific Personnel/Employee Relations Responsibilities: Assign duties to non-licensed nursing staff and supervise charting activities so patient records are complete and a accurate. Supervise activities of non-licensed nursing staff so policies are followed. Carry out personnel disciplinary actions to include issuing oral and written personnel in instances where he/she believes company employment policies, rules for conduct or accepted patient care practices have not been followed. Resolve employee work related problems, complaints, and grievances to extent possible. The first Line supervisor is the first step in the formal compliant procedure described in the Personnel Manual. Assist in orientation for new non-licensed nursing staff and recommended their promotion from probationary period to regular employment status. Evaluate work performance of non-licensed nursing staff. Make changes in staff schedule as indicated by absence or illness and obtain replacement where needed, Attend appropriate nursing supervisory meetings. Willing to perform duties as a C.N.A when needed. Certified Nursing Assistant (CNA)- Purpose: Provide direct and indirect patient care activities under the direction of a Registered Nurse or Licensed Practical Nurse. Assist patients with activities of daily living, provide personal care, comforts and assist in the maintenance of a safe and clean environment for an assigned group of residents. Qualifications: Health Ability to meet performance requirements Educate and Training Must be Certified Nursing Assistant Attend in-service programs, as assigned, to learn procedures and develop skills and meet state requirements Personal Qualifications: Sensitive to our residents physical and psychosocial needs. Specific Duties and Responsibilities Required: Understand and practice universal precautions with all resident care Perform all task/Procedures included on assignment or report to charge nurse any task not completed. Assists in the preparation for admission of residents Assist in and accompanies residents in admission, Transfer, and discharge procedures (Packing, unpacking, etc) * Provides post-mortem care Provide morning care, which may include bed bath, Oral Hygiene, combing hair, back care, dressing residents, changing bed linen, cleaning over-bed table and bedside stand, straightening room, and other general care as necessary throughout the day. * Provide evening care, which may include washing hands/face as needed, oral hygiene, special skin care, freshening linen, cleaning over-bed tables, straightening room, and other general care as needed. * Provides general nursing care such as positioning residents, Lifting and turning residents, Applying/utilizing special equipment, assisting in use of bedpan, urinal or commode, and ambulating the resident with a gait belt. * Takes and records temperature, pulse, respirations, weight, height, blood pressure and intake and output measures * Adheres to policies and procedures of the center and the Department of Nursing. Participates in socialization activities on the unit and assists residents to activities. Tums and positions as ordered and/or as needed, making sure no rough surfaces are in direct contact with the body. Lifts and turns with proper and safe mechanics and with available resources. Checks for reddened areas or skin breakdown and reports to an RN or LPN Ensures that residents are dressed properly and assists as necessary. Ensures that clothing is properly stored in dressers or on hangers. Ensures that all residents are clean and dry at all times * Check unit for adequate linen and supplies Restrains residents properly when ordered. Understands reasons fro restraint and suggests less restrictive measures if possible * Accompanies residents to appointments as directed Provides reality orientation in daily care Prepares residents for meals and services, removes food trays and assists with meals or feeding residents * Distributes drinking water and other nourishments to residents Performs general care activities for residents in isolation, adhering to the center’s Infection Control Policies Answers residents call lights, anticipates residents needs, and make rounds to assigned residents Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aides, and prosthetic devices) * Collects and transports specimens as directed Reports (and records when appropriate) and changes observed in condition or behavior of resident and any unusual incidents * Familiar with Nursing Care Plans and contributes when new information is available. * Assists in maintaining a safe and clean environment, reports equipment that is out of order, reports other problems to the charge nurse, maintains appropriate handling of clean and soiled lined, and stores supplies on the unit. * Establishes and maintains interpersonal relationships with residents, family members, and other center personnel, while assuring confidentiality of resident information * Attends in-service education programs, as needed, to learn new procedures, develop skills, etc * Practice careful, efficient and non-wasteful use of supplies and linen, and follows established charge procedures for resident charge items. recblid b0gyfacu3tf28n435tnmeh1ehhswn1... - Permanent - Full-time
