The Referral Management Specialist is the clinician responsible for the oversight of all weekend referral activity. This oversight includes (based on received documentation) patient eligibility for skilled home health services and determination of homebound status. This position also makes an initial determination of environmental safety which may include physical barriers at the residential site or safety issues that may negatively impact patient care and clinician safety.
