Establish and maintain positive working relationships with referring physicians, social workers, and dialysis centers. 2. Promote positive community relations through development, coordination and promotion of special events, speaking engagements, and educational activities that support the hospital and transplant center's mission. Coordinate community outreach activities. 3. Facilitate the creation and distribution of transplant patient education material in collaboration with Transplant Services Director, Parkland Education and Marketing. 4. Serve as the transplant program liaison for social workers, nurses and other clinical support staff that interact with pre and post-transplant patients. 5. Continuously works with referring dialysis centers and social workers on new patient referrals, including obtaining all prevalent patient information: Kidney and Pancreas Pre-screening checklist, intake form, physician history and physical, recent laboratory test results, copies of medical insurance information, etc. 6. Participate with satellite outpatient clinics to further develop relationships with referring physicians and patients. Other duties as assigned. Contributes to the development of goals and planning efforts for the department. May manage large projects or processes that span outside of the department. Knowledge & Experience Requires mastery of specialty area and working knowledge of industry practices/other areas, typically obtained through advanced education and strong experience. Degree Requires theoretical knowledge, often gained through a combination of experience and/or advanced education, typically college / university degree Bachelor's.
